All adult Monteggia fractures require urgent/emergent surgical treatment.

Pediatrics:

• Monteggia fractures may have a trial of closed reduction under general anesthesias before moving to open reduction.
• The radial head and capitellum should line up in ALL views; any malalignment should raise suspicion for radial head dislocation.
