Most scapular body fractures are non-operative and most glenoid fractures require surgery.

Surgical treatment for the following injury patterns:
– Intra-articular fractures
– Glenoid fractures with >25% surface involvement or 5mm displacement
– Glenoid neck fractures with other associated fractures or >20° inferior angulation
– Comminuted scapular body fractures
– Displaced acromion fractures
– Coracoid fractures

Urgent surgical evaluation for:
– Floating shoulder (scapular neck fracture with associated clavicle fracture or ligamentous injury)
