Pearls

• Displaced fractures require surgery.
• Reduction under sedation can be attempted in the ED, but most patients will require surgery.
• CT scan is frequently needed to characterize full extent of injury.
• Non-displaced talar body fracture can be treated nonoperatively.

Complications

Most patient will develop arthritis and are at a high risk of AVN (58% of patients in one series).
